Your company’s healthcare scheme includes insured members who are either Swiss or who are based in Switzerland. To cater for these members, we have recently launched our new Suisse International Healthcare Plan, a locally compliant healthcare solution designed to cover your employees in Switzerland as well as abroad.

Important information in accordance with Art.3 VVG

As you are the designated Group Scheme Manager within your company, please find below the three easy steps that you will have to complete. These will allow us to successfully incept your scheme’s cover under the Suisse International Healthcare Plans:

By the last working day in November:

  1. Check if your company has any supplementary health insurance contracts with another insurer and cancel it, to allow us to set up the new cover under the Suisse International Healthcare Plans.
  2. Gather the application forms completed by all employees to be insured and return them to us at: group.admin@allianzworldwidecare.com stating the following in the subject line of the email: Swiss Application Form. This will enable us to incept the cover of the employees.
  3. Inform the employees to be covered that they will need to cancel any LAMal/KVG cover that they may have, by no later than the last working day in November. They will also need to complete the Bank Set-up Form and send it to their Swiss bank.

Important information in accordance with Art.3 VVG

Please find below the three easy steps that you will have to complete to allow a successful inception of your cover under the Suisse International Healthcare Plans:

By the last working day in November:

  1. Cancel any existing LAMal/KVG cover you may have (a cancellation letter template is provided here for your convenience, if you wish to use it). You will also need to settle any outstanding amounts with your current LAMal/KVG insurer.
  2. Complete the KPT Application Forms (one for you and then one per each dependant you are including in your cover) and the Allianz Application Form. The KPT Application Forms are for your LAMal/KVG insurance inception; the Allianz Application Form is for your supplemental and international insurance. Please return your completed forms to your company – they will forward them to us for your inception.
  3. Complete the Bank Set-up Form and send it to your Swiss bank.
