Claim back your eligible costs via our MyHealth app or online portal. Simply enter a few key details, add your invoice(s) and press ‘submit’.
As an alternative, you can also complete one of the forms below. If you are uncertain which claim form to use, please contact our

If you applied for your policy independent of an employer and pay your own premium, you are an individual member. Whereas underwritten groups typically consist of 3 to 9 members.
