Pre-authorisation Process for Medical Providers

 
  • In-patient or day-case treatment
  • Out-patient surgery
  • Chemotherapy and radiotherapy
  • Medical evacuation & medical repatriation
  • Repatriation of mortal remains
  • Rehabilitation
  • In-patient maternity

Please note that some policies may require pre-authorisation for other treatments.  Please log in to Online Services to check the patient’s Table of Benefits   


  • Planned admission/service or treatment date
  • Diagnosis, including ICD9/10 code  (if unknown, please state provisional diagnosis)
  • Planned procedure/treatment, including CPT/CCSD code
  • Estimated costs and currency
  • Medical report or any other relevant medical document available

Submit the information and supporting documents to: medical.services@allianz.com. 

Please send us the request at least 5 working days before the date of service.

If urgent (within 72 hours of the treatment) please contact us to flag the case to our Medical team for priority handling.
